Traditional Format

 連載第1回において,口頭症例提示はIdentification & Chief ComplaintsとHistory of Present Illnessにはじまり,Assessment & PlanとConclusionに終わる,すなわち,Contents Indexに表示してある順序に従うべきことを強調しました。そして,連載第2回から第9回まで,各項目について詳論してきました。

 この方式に従った口頭症例提示を,Traditional format(Conventional format)と呼ぶことにしましょう。Traditional formatの長所は,鑑別診断と治療方針に関する徹底的な議論を促す点にあります。入院診療(一般病棟)の場に最もふさわしい形式であると同時に,clinicopathological conference(CPC)や学会発表への応用も可能です。

Traditional Formatの限界

 しかしながら,Traditional formatがすべての状況において万能であるとは限りません。外来やemergency room(ER)では,Traditional formatは議論のための議論を生み出す傾向にあり,診療が効率よく進まないという弊害もあります。また,intensive care unit(ICU)においては,臓器系統別の全身管理に焦点が当てられていることから,現病歴以外の病歴が果たす役割は最小限であることが多いものです。

 したがって,日々の動線であるER→ICU→general ward(病棟)のそれぞれの状況にふさわしい口頭症例提示法が模索されなければなりません。Traditional formatの枠外に,一歩踏み出してみましょう。

ERにおける症例提示

 ERでは,確定診断も大切ですが,それよりも患者の全身状態の安定を優先しなければならないことが常です。また,ある患者を診ながら,別の患者について報告を行うといった状況も日常茶飯です。

 このような環境では,口頭症例提示も,ズバリと核心をついた簡潔なものでなければならず,ただちに次の一手に結びつき,患者の救命と生存に役立つものでなければなりません。

Assessment-Oriented Format

 ERおよび外来にふさわしい症例提示法の1つとして,Assessment-oriented format(AO format)があります。表1にその概要を示しました。

(4)以上を根拠づけるために必要最小限の病歴,身体所見,検査所見。
【コツ】
(2)と(3)が最も重要です。2分以内を目標とします。

 どのような患者が来院しているかを述べたら,直ちに結論,すなわち,診断と治療方針を述べます。Traditional formatに慣れている精神にとって,この飛躍は,勇気がいることでしょう。しかし,AO formatこそ,重要なことを一番最初に述べるという戦略の実践に他なりません。いったん慣れてしまえば,非常に快適です。

 その後,時間の許す範囲で,(4)根拠となる病歴,身体所見,検査所見を,因果関連を明瞭にしつつ,報告します。この時,(4)の中で,(2)と(3)を繰り返してしまいがちなので,それを避けます。(4)の分量に一定の決まりはなく,指導医が次々と質問を交えてくる場合もあれば,緊急性が高い場合は,全部省略して,直ちに患者の診療に向かうこともありえます。

 症例提示時間は,全体で1-2分を目安とします。

 Text Boxに,交通事故後の左血気胸の症例を掲げました。最後から2番目の段落でpertinent positiveを,最終段落でpertinent negativeを述べています。

 実際にAO formatを導入する場合,teacherとlearnerの双方が,その構造と目的を熟知している必要があります。Maddow CLらの論文(文献1)を全員が一読し,さらに,以下の3点に留意することをお勧めします。

1)指導する側もされる側も,ともすればtraditional formatに回帰しがちですが,AO formatはtraditional formatの縮小版ではありません。項目立てが異なります。
2)AO formatはthorough presentationではなく,次の一手を決めるためのfocused presentationです。
3)AO formatによる症例提示後に,発表者は必ずpreceptorと共に患者を診察し,double-checkを行います。

 Assessment-Oriented FormatによるER Presentationの例

This is a 17-year-old female, high school senior, brought here by ambulance after a motor vehicle accident.

Working Diagnoses: Broken ribs and related chest wall muscle contusions, a left-sided hemopneumothorax, and a very severe intrathoracic bleed that led to hypovolemic shock and acute metabolic acidosis with respiratory compensation.

Assessment: The patient's condition has been successfully stabilized by administering intravenous fluids and a blood transfusion. While inserting a chest tube into the left thoracic cavity, a large volume of blood, approximately 1.5 liters, and some air is collected from the chest tube. We need to identify and repair the site of bleeding.

Plan: Take the patient to surgery for an exploratory thoracotomy. Place a Foley bladder catheter to monitor her urine output.

History very briefly: The injury arose from the deceleration accident. She drove after drinking and swerved into a concrete wall. The left front side of the car smashed and the steering wheel bent. She was not wearing her sheet belt. The windshield was intact. She continued to complain of left-sided chest pain and progressive difficulty breathing.

Important findings: Her vital signs were remarkable for a respiratory rate of 34, an increasing heart rate 120 at the accident site to 150 in the ER and a steadily dropping blood pressure from 170/100 at the accident site to 90/60 on arrival at the ER. She was pale and in obvious distress. She had smell of alcohol. Asymmetry of chest wall movements with breathing, bruising, crepitus, and subcutaneous emphysema of the left anterior chest wall were noted. There was dullness to percussion of the left side of the thorax. Auscultation revealed markedly decreased breath sounds on the left. Her arterial blood gas revealed: pH 7.32, PaCO2 32, PaO2, 75, HCO3 16, oxygen saturation 92% under room air.

Other information: Her extremities felt cold to the touch, but there were no signs of traumatic injuries to her arms or legs. The remainder of her examination, including the head, neck, abdominal, musculoskeletal and neurological examination, was normal. The result of a cranial CT scan was intact. Urine was clear. She was previously healthy. No medications, no allergy, no drug abuses. Menstruation was regular and she denied a possibility of pregnancy.

Politically Correctness

 International normとは,場の多様性を理解し,文脈を読み,その状況に柔軟に対応していくための行動規範です。その1つとして差別表現(discriminating language)を撤廃し,中立的表現を採択していこうとする態度Politically Correctness(PC)があります。

 実際にはありえない極端な例ですが,「盲目の日本人」をblind yellow Japと切り出されたら,私たち日本人は居心地がよくありません。医学的な文脈では,blind Japaneseと修正するべきでしょう。しかし,blindという単語には,日本語の「めくら」に通じる響きがあります。本人または家族が目の前で言われたら,傷つくでしょう。Blindの代わりにwith total blindnessあるいはvisually impairedなどのように語形・語法が工夫される所以です。

差別が解消される文脈

 ところで,「盲目」の「黒人」ミュージシャンStevie Wonderが自分自身をblind blackと語っていますが,この場合,誰が彼を非難するでしょうか。また,白人が自己卑下的に「white person」という語を用い,会場をユーモアの渦に巻き込むこともあります。このように,ある表現が差別的であるか否かは,誰が誰にむけて,どういう文脈でその表現を発したかという状況に強く依存します。

 表2にgender, race, physical or mental disability, social statusに関するPolitically Correctnessの例を掲げました。このような語法に普段から注意を払い,覚えていく努力をしておくと,様々な場面で役に立ちます。

♪間奏曲

 「Hippocratesの誓い」を持ち出すまでもなく,患者のプライバシーを守ることは,診療行為の一部です。米国でも,患者情報保護は,連邦法により規定されてきました。一番最近では,the Health Insurance Portability and Accountability Act of 1996(HIPAA)において,電子媒体も含めた患者情報の取り扱いが再規定され,2003年4月14日から施行されています。HIPAA全文を読む時間はないとしても,文献2に目を通しておきましょう。
