 連載も前半6回を終え,後半に突入です。深呼吸してリラックスしたら,新しい一歩を踏み出しましょう。夏休み,学外・海外研修などを経て,心機一転,臨床医学への邁進を決意なさった方も多いはずです。その熱意を応援します。

Aha Experience

 英語を母語としないことに劣等感を感じている日本人が多い中,多民族国家・米国に住んでいると,そんなことはまったく問題ではないことに思い至ります。この国では,英語しか話せない人はむしろ肩身が狭い思いをします。

 ある時,fourth-generation American of Japanese ancestry(日系四世)の同僚Jill(Dr. Omori, Family Medicine)が,日本人医師を対象としたfaculty development workshopの打ち上げの後,筆者に向かってしみじみと語りました。

 “You speak both English and Japanese. I envy you. I only speak English, though someone expects me to speak Japanese because of my appearance."

 言語は人間存在の根底を支配しています。英語しか話せない人より,英語話せる人のほうが,より広い思考の可能性に開かれているといえます。日本語を母語としつつ,英語を学ぶことは,劣等でも弱点でもなく,恩恵なのです。

BSPE

 Physical examinationには,診察,診療録記載,症例提示,いずれの場合にも共通した一定の順序があり,これをbasic sequence of physical examination(BSPE)と言います。表1を徹底的に暗記してください。

表1 BSPEの呪文
Begin with general appearance and vital signs, then skin, then proceed from head to toe, and conclude with the musculoskeletal and neurological examination.

 症例提示に特有の注意点は2点です。

1)言及項目は,特に完全記述を指定された場合以外は,診断と治療方針の決定に関連する陽性所見(pertinent positive)陰性所見(pertinent negative)のみに絞ります。

2)議論に関係しない臓器系統(system)はnormalまたはunremarkableと簡潔に言い切るか,省略します。

 日本語の発想からは生まれてこない英語圏における常套表現を,Text Boxに集約してみました。完全でも包括的でもありませんが,これらの表現をまず覚えておくと,身体所見に関するその他の英語表現を磁石のように吸い寄せていく核となります。だまされたつもりで,可能な限り暗記してください。HEENTはhead, eyes, ears, nose, and throatの略で,ヒーントまたはエイチ・イーエヌティと読みます。

身長,体重,体温

 Global standardという虚構を心拝し,かつ,それを米国と同一視する二重の過ちを犯している日本の風潮とは裏腹に,米国人は外部の世界にひどく無関心で,自己の慣習に固執しがちです。度量衡もその一例であり,cm,kg,Celsiusを使いこなせる米国人は一部の知識階級にとどまり,feet,inch,pound,Fahrenheitに慣れ親しんだ平均的な米国人は,両システムの換算を知らない有様です。

 しかし,ここはひとつ,文句を言わず,両方のシステムを把握し,自由に換算できる軽快な知のフットワークを身につけましょう。もちろん,いちいち計算機を取り出していては,臨床は進みません。表2を暗記して,スマートに頭の中で概算できるようにしましょう。

Eye Contact

 ハワイ大学医学部で,アイコンタクトが十分でない学生に対して,例えば,説明のためにホワイトボードに気を奪われている場合,筆者は“Don't talk to the board.”と短く注意します。参加者とcommunicationすることがpresentationの目的だからです。

 私たち日本人は,聴者と視線を合わせたがらない民族です。合わせたとしても,視線の力が極端に弱い。集団の中で,自己主張せず,ひたすらうつむいてやり過ごすことが日本社会の伝統的な規範だからでしょう。翻って,欧米においては,アイコンタクトの不足は,熱意のなさ,自信のなさと解釈され,能力のない人と評価されます。

 アイコンタクトの秘訣は以下の3点です。

1)可能な限り長い時間を 目標:全発表時間の90%以上。

2)可能な限り力強い視線で 熱意,話す内容に関する自信,話す行為に関する熟練,すべてを込めます。

3)参加者の1人ひとりに平等に注ぐ 聴衆が大勢の場合は,全方向に均等に。

 これらの秘訣は,採用面接や企画会議においても重宝します。自分の意見を,相手の目をまっすぐに見つめ,最初から最後まで視線をそらさずに述べるのです。そうすることにより,相手の目の中に宿る真実を見逃すこともなくなり,物事の進展と決済が驚くほどはかどるようになるでしょう。

Facial Expression

 発表者の自信も不安も,誠実もごまかしも,すべて表情に出ます。「顔に書いてある」という言葉通り,顔面表相は取り繕うことができません。一般的な注意点として,状況にふさわしい表情を心がけます。ニヤニヤしない,薄ら笑いを浮かべない,などです。

 顎を引けば,冷静沈着な印象を与えます。強調したい場合は,両目をむく,顎を突き出す。確認を求めたい場合は,全員を見回しながら,うなずくように大きく首を上下に振る。慣れて上手に使えば,すべて有用です。

Physical Examinationの提示に役立つ重要表現
【This is a 58-year-old man admitted to the ICU because of aggravating dyspnea.】
Physical examination reveals an anxious and diaphoretic man looking older than his stated age, who is in moderate respiratory distress, using his accessory muscles to breathe and sitting upright. He is 170 cm tall, weighs 88 kg.
Vital signs: Temperature, 38.2C, orally; blood pressure, 180 over 80 in both arms with orthostatic changes of 15mmHg; radial pulse, 95, which is irregularly irregular. Respirations 20, labored. His breath has a particular odor.
Skins: No jaundice, cyanosis, but several spider angiomas on the upper trunk and palmer erythema are noted. Nail beds are pale. There is a tattoo on his left arms.
HEENT: Supple neck in response to flexion, extension, or rotation. The tympanic membranes and external auditory canals are clear with normal landmarks. Extraocular movements are full. Funduscopic examination reveals papilledema, moderate narrowing of retinal arteries, and arteriovenous crossing changes, without hemorrhages or exudates. The pupils are 3.0mm in diameter, equal, and react directly and consensually to light. Sclerae are anicteric, but conjunctivae are pale. He has a clear oropharynx and good dental hygiene. No cervical bruits, thrills, or adenopathy. The jugular venous pressure is 7cm above sternal angle. The trachea is midline and movable.
Chest: Bibasilar rales and diffuse mild wheezes are present. Chest wall is nontender.
Cardiac: Normal S1 and physiologically split S2. A 2/6 systolic murmur, loudest at the left upper sternal border and S4 at the apex are present. No S3, diastolic murmur, or rubs. The point of maximal impulse(PMI)is displaced downward and laterally.
Abdomen: Soft and flat. Bowel sounds are present but reduced. A smooth, soft, nontender, nonpuslatile liver felt 3 cm below the right costal margin in the midclavicular line. No splenomegaly, or demonstrable ascites. No bruit or aortic enlargement.
Back: No costovertebral angle tenderness in response to fist percussion. No scoliosis.
Genitourinary: Normal male genitalia.
Rectal Examination: No evidence of sphincteric incompetence. A homogenous prostate gland is slightly enlarged. The stool is negative for occult blood.
Extremities: Peripheral pulses are all present and symmetrical without bruits. Moderate pitting edema extending up to her both knees. No venous cords, calf or thigh tenderness, or clubbing.
Mental Status: Alert and oriented to person, place, and time.
Neurologic evaluation: Cranial nerves II through XII are intact. Peripheral sensation is decreased to light touch and vibratory testing in his hands and feet. Movement of all extremities is full, forceful and symmetrical. No asterixis. The deep tendon reflexes are diminished throughout but symmetrical, with downgoing toes(plantar flexion of his toes).
