最初に最終目標

 みなさん,はじめまして。このコーナーは,医師としての情報発信(「しゃべり」)の力を鍛えていくための新連載です。「日本人医師(医学生)が,英語で臨床症例を提示し議論を行なう」という場面に対する米国発日本初の包括的マニュアルとなります。「米国のクリニカルクラークシップに参加する」「英語圏で臨床研修を行なう」といった目標設定にも直結しています。
 早速に「エーイーゴー」という声が聞こえてきそうですが,新しい時代を担う医師にとって英語は必須。むしろ「英語が苦手」「留学なんてあり得ない」と考えている人にこそ最大の契機となる記事をめざしています。臆せず気軽に読み進んでください。
 「たかが症例提示」と侮ってもいけません。「ConferenceにはじまりConferenceに終わる」Communication重視の米国医学教育において,症例提示は「空気のように重要」です。多くの指導医が,能力評価のうえで最も重要な指標と考えていることも事実です。その理由は,次の3つです。
1)優れた症例提示を構成するためには,症候学,診断学から病態生理,疾患各論に至るまで,幅広く深い知識が必要です。
2)それらの知識により学者にはなれますが,臨床医であるためには,医療面接法と身体診察法の双方に習熟したうえで,良好な患者-医師信頼関係を築き,的確に情報収集を行なう技能が必要です。
3)以上の知識と情報を整理して他者に伝達するためには,完成されたコミュニケーション技術が必要です。 どの要素も一朝一夕には成り立たず,生涯謙虚に学習していく必要があります。
 翻って考えてみるに,私たち日本人は,情報発信が不得手,かつ英会話が不自由という「二重苦」,さらに,その「二重苦」に無自覚であるという「三重苦」を負っているので,本当に奮起しないといけません。「もう一度あの人の話を聴きたい」と思わせる「しゃべりのプロ」になるために,表1-1のように目標を設定しましょう。一年後の私たちの「仕上がりの図」です。

症例提示

 早速,現場を覗いてみましょう。ハワイ大学医学部では,模擬患者の問診と診察を行ない,その結果を第一線の臨床医の前で口頭提示する教育が,入学直後からはじまります。3か月も経過すると,素人から医師へと急速な変貌を遂げている彼らの頼もしい姿に接することができます。初回ですから,私たちも医学部1年生のこの水準から入門しましょう。Text boxの症例「Phil Collins」を大きな声で音読してください。

構成内容

 症例提示の内容(Contents)は,Contents Indexに示したBasic Sequence(12項目)に従います。マニュアル絶対視は真の知性とは言えませんが,初心者はこの順序を遵守することに専念してください。熟達すれば,省略や順序変更を行なっても説得力を失わない自由を獲得できます。
 最初にIdentification Data(ID)とChief Complaints(CC)を元気に一息で述べた後,History of Present Illness(HPI),Past Medical History(PMH),Family History(FH),Social History(SH)と一連のHistoryを丁寧に述べます。Review of Systems(ROS)により問題点の見落としがないことを確認した後,Physical Examination(PE)とLaboratory Findings(Labs)により症例に客観的な彫りを加えます。
 「Phil Collins」は,医学生が問診と身体診察の結果を指導医に報告したものなので,Labsはありません。以上ID & CCからLabsまでの情報を素早く整理し再確認するのがSummaryです。これにより,後続のProblem Listの立案理由とAssessment & Planの内容が理解しやすいものとなります。最後に,症例に対する主治医の見解をきっぱりと述べてConclusionとします。
 実際の発表で完全無欠を達成することはなかなか困難です。表1-2を見てください。構成内容に関して,私たち日本人に共通する弱点です。英語症例提示に限らず,日本語で発表を行なう場合でも,これらの弱点を修正するだけで,知的水準の印象が見違えて向上します。これらの弱点をどう克服するかについては,次回以降の各論で詳述します。

■Delivery Index

伝達技術

 伝達技術(Delivery)は内容と同じ重要性を持つにもかかわらず,日本の臨床医学においてその系統教育は不在です。「伝え方が下手であれば,どんなに素晴らしい内容も無に等しい」と肝に銘じておきましょう。
 Delivery Indexに,私たち日本人が特に配慮すべき10項目を示しました。Mindsetは,発表の「心構え」です。Deliveryの最大の焦点は「話し方」ですが,これを速度(Pace),発音(Pronunciation),声調/声量(Pitch & Volume),間合い(Pause)の4項目に分割すると課題が明確になります。
 発表において私たちは音声的存在であるばかりでなく,身体的存在でもあります。日本人は特に「動き」に乏しいので,Eye Contact & Facial Expression(視線/表情)とPosture & Gesture(姿勢/身ぶり)を活かす努力が大切です。
 症例提示の上級者になるためには,弛まぬ語彙(Vocabulary)構築に加えて,国際社会の多様性に対する日本社会の閉鎖性に由来するコミュニケーションの齟齬にも配視しなければなりません。国際社会における常識とタブー(International Norm)を理解し,日本国内で無自覚に使用されている和製英語(Japanese English)の弊害を意識することが第一歩となります。

練習-2
1)Deliveryを構成する10項目を覚えてください。
2)症例「Phil Collins」について
 (a)自分の自然な速さで音読し,時間を記録してください。
 (b)5分で音読してください。
 (c)原稿を暗記し,5分で発表してください。

総合指針

 たいへんな駆け足で,構成内容と伝達技術を俯瞰してきましたが,これら全項目が調和したところに,骨子がはっきりした(well-organized)理解しやすい(easy to understand)症例提示が生まれます。このための指針として,英語圏における症例提示教育において頻繁に使われる「誉め言葉」を覚えておくと,背筋も実力も伸びます(表1-3)。明らかに意味の重複が存在していますが,各論の森に迷った時,常々立ち返る総合指針の1つとしていきましょう。

♪ 間奏曲

 以上で一緒に勉強していくための枠組みが完成しました。医学でhop,英語でstep,症例提示技術でjump。一歩と言わず,三歩以上差のつく医師をめざしてがんばりましょう。困難に思える目標も,小さく分割して取り組めば難しくありません。次回から各論です。
 日本にいながら英語症例提示を学ぶことは本当に可能でしょうか。答えはYesです。あなたが個人(Individual)を自覚しさえすれば,あなたはいつでも学びの世界の中心だからです。Aim high! You can do it.

Phil Collins [Based on the PBL tutorial(HCP9/Unit1.15)in the UH/JABSOM MD Program.]

Mr. Phil Collins is a 62-year-old homeless Caucasian male who came to our clinic for the first time complaining of a two-week history of right foot pain.

HPI. The patient was in his usual state of health until approximately two weeks ago when he accidentally stepped on the broken bottle and cut his right heel on a piece of broken glass. The pain gradually worsened and now he can't bear any weight. He described the pain constant and throbbing and rated the pain 7 out of 10. He has noticed that the area of redness and swelling had started in the immediate area around his wound and was still increasing so that now his calf was involved.

He wasn't able to give us any information about his past medical history. However, he did admit he was hospitalized at Hawaii State Hospital a few times, but he didn't know why. He used to take medications from a psychiatrist. However, he stopped going to see him and hasn't seen any physicians for the past two years. He denied any allergies, and he doesn't remember anything about immunizations or childhood illnesses. He couldn't tell me anything about his family history, and in fact didn't know about where his family was. His social history is only significant in the fact that he is homeless. He denied drinking, smoking, or any type of drug use. He spends most of the time at Ala Moana Beach Park. Review of systems is unremarkable. But again I wasn't quite sure if I understood well when I was asking him.

Physical examination. Generally, he was a thin white male, appearing slightly older than his stated age, with very poor personal hygiene. He was alert and cooperative. He favored his right leg. His vital signs were temperature 102 F, heart rate 90/min., regular, respiration 18/min., unlabored, and blood pressure 130/88 mmHg. As for the lower extremities, it revealed a 3-cm wide, shallow ulcer with a necrotic base, and some purulent discharge on his right heel. There was erythema, swelling, and tenderness of the right foot and calf. His pulse on femoral, popliteal, dorsalis pedis, and posterior tibialis, was intact. He also had some tender palpable lymphadenopathy in the right inguinal area. Neurologically, his right leg was normal. The rest of his physical examination was normal.

In summary, this is a 62-year-old homeless Caucasian male with a history of unspecified psychiatric illness who presents two-week progressive right foot pain and now calf pain. His examination is significant only for a temperature of 102 F, a right heel ulcer with associated swelling, erythema, tenderness, and again inguinal lymphadenopathy.

Problem list: #1 Right foot injury, #2 History of psychiatric illness, and #3 Homelessness. Assessment and plan: #1 Right foot injury. The wound requires immediate medical attention. We would obtain a culture from the ulcer, order CBC and X-rays of the foot, perform daily debridement by a surgical consultation, place him on intravenous cefazolin, and give a tetanus vaccination booster. #2 History of psychiatric illness. We assume that he had been treated for chronic schizophrenia. We would perform a mini-mental status examination and then ask a psychiatric consultation. #3 Homelessness. We discuss the issue of placement with our medical social workers.

My impression is that Mr. Collins has a very severe cellulitis, possibly with the involvement of the underlying bone as well. We would arrange for his admission to our hospital.
