4月に最終回

 4月,新しい教育年度の始まりです。本紙を新しく手にする読者も多いと存じます。本欄では,「英語による口頭症例提示」を題材とし,「医師として,世界に向けて情報を発信できる」ことを目標にして,2004年1月から連載を続けてきました。今回が最終回です。

学びの環

 4月に最終回とは幸運な巡り合わせです。連載の全容を最初から把握できるからです。Begin with the end in mind! 初めに目標ありき。明確な目標がなければ,どこにも到達することができません。今回の記事は全体の要約なので,最良の出発点でもあります。読み終えたら,直ちに第1回にお進みください。

 英語による症例提示に限らず,プレゼンテーションは,世界に飛翔していこうとする医師にとって必須の技術です。End with the beginning! 終わりは新たな始まり。学びの環には,始まりもなく,終わりもありません。本稿では,随所に過去記事への参照を挿入しました。全12回の内容を「批判的に吟味」していただければ幸いです。では,始めましょう。

Contentsのまとめ

(1)内容と順序
 Contents Indexを左から右にご一読ください。この12項目はbasic sequenceと呼ばれ,口頭症例提示の最も標準的な構成内容です。今一度,しっかり覚えてください。第1回において,各項目の意義を俯瞰しました。第2回から9回まで,各項目について解説を行いました。入院診療(一般病棟)の場で,時間をかけて症例を検討する場合に,最も好まれる形式で,Traditional(またはconventionalformatとも呼ばれます。

(2)変法
 施設や診療科が異なれば,症例提示のスタイルも異なります。所属施設の習慣を絶対視することなく,出会ったスタイルを柔軟に習得していきましょう。代表的な変法として,AO formatとICU formatがあります。救急診療や外来診療の場で威力を発揮するAO(assessment-oriented)formatは,最初に診断と治療方針を述べ,その後,必要事項を補足していく方法です(第10回)。ICU formatは,重症患者について検討する際に有用で,臓器系統別に(by system)所見を述べていく点が特長です(第11回)。

日本人のための特記事項

 日本の医学生,研修医を指導していて痛感することは,症例提示の基本12項目が「身について」いないということです。「えーと,えーと」と考えながらであれば,順番を思い出すことができるのですが,「反射的に出てくる」水準までは,鍛錬されていません。

 原因は2つです。第一に,口頭症例提示の経験数が絶対的に少ないのみならず,患者の問診と身体診察の経験数が不足しています。症例提示の流れは,実は,診療の手順そのものでもあるので,診療に主体的に従事していれば,症例提示の構成内容にも迷いはないはずです。第二に,指導医自身が,診療においても,症例検討においても,一定のformatに従うことの威力と能率改善に無自覚なため,後輩に対して有効なお手本を示すことができていません。

 今一度,症例提示の12項目をreviewし,下記のtext boxでその具体例を確認してみましょう。臨床医にとって,症例提示の基本形は,空気のように自明でなければなりません。

Traditional FormatによるPresentationの一例[Based on the PBC (HCP5/Unit1.15), UH MD Program.]

[ID/CC]
Our next patient is a 27-year-old Caucasian male, high school math teacher who presents with 2 days of right upper quadrant(RUQ)abdominal pain and fever. This is his first visit to our clinic.

[History of Present Illness]
He first started feeling like he had “the flu" about 10 days ago, with symptoms of body aches, fatigue, anorexia, mild nausea and “feeling hot". Two days ago, he developed worsening RUQ abdominal pain, fevers to 103.0 degrees F, yellowing of his skin and eyes, and diffuse pruritus. The pain is constant and does not radiate to anywhere else in the abdomen. He also noted darkening of the color of his urine. He did not eat any raw foods recently. No recent travel.

[Past Medical History]
He has been healthy. No known allergies. He denies any major medical problems, and is not on any medications. No history of surgery. No transfusion. Not received the immunization series for hepatitis B.

[Family History]
Negative for liver diseases or alcohol abuse.

[Social History]
He drinks an occasional glass of wine with dinner and never uses any illicit drugs. He does not smoke. He is homosexual. He and his partner have been in a monogamous relationship for the past 2 years. They don't use condoms. HIV negative when they were last tested two months ago.

[Review of Systems]
He denies having vomiting, diarrhea, joint pain, weight loss, cough, sputum, or hemoptysis.

[Physical Examination]
Vital signs: Oral temperature 101.8. Respiratory rate 16, unlabored. BP 132/80 sitting. Heart rate 88, regular. Height 6'0". Weight 178 lbs. General appearance: physically fit male appearing his stated age. Alert and fully cooperative. Skin: Moderately jaundiced. No palmar erythema or spider angiomas. No tattoos. Sclerae and oral mucosa icteric. No Kayser-Fleischer rings. The lungs and heart were normal. Abdomen: Flat. No scars. Bowel sounds normoactive. Moderate RUQ tenderness to palpation. Liver span percussed to 15 cm in the right midclavicular line. Spleen not palpable. Rectal examination with brown stool, guaiac negative. Genitalia: Normal circumcised penis without discharge or ulcerations. No edema on extremities. The remainder of physical examination was normal. A neurologic examination showed no abnormalities.

[Laboratory Test Results]
CBC: WBC count was on the high end of the normal range of values. No atypical lymphocytes. Hb, Hct, and Plt: normal. Liver function tests: markedly elevated AST, ALT and total bilirubin 3.5 mg/dl. Alkaline phosphatase(ALP)is two times normal. Electrolytes, blood urea nitrogen, creatinine, glucose, and amylase levels were normal. Hepatitis serology: positive HBs-Ag, positive HBe-Ag, positive HBc-Ab, negative HBs-Ab, negative hepatitis delta antibody. Tests for hepatitis A antibody and hepatitis C antibody were negative. Prothrombin time: mildly prolonged. Urinalysis revealed 2+ bilirubin and urobilinogen negative. Chest film, ECG: normal. Abdominal film did not reveal calculi, air in the biliary tree, or ileus.

[Summary]
This is a 27-year-old male who presents with acute RUQ abdominal pain, jaundice, and fever. Blood test revealed a positive profile of hepatitis B. He is homosexual and has a monogamous relationship with his partner.

[Problem List]
All the problems could be discussed based on the working diagnosis of acute hepatitis B.

[Assessment & Plan]
# Diagnosis
RUQ localization of the pain, jaundice and fever in this age group help us focus on infectious hepatobiliary etiologies such as hepatitis, liver abscess, or cholecystitis. Constant pain suggests intrahepatic process rather than extrahepatic obstruction. The physical examination findings are consistent with acute hepatitis, while drug-or alcohol-induced hepatitis is unlikely because of the history. An acute onset without weight loss makes cancer less likely. The “AST > ALP" pattern would suggest hepatocellular injury rather than cholestatic abnormalities. The hepatitis serology confirms that the patient has hepatitis B infection and currently still be “infectious". We need to report his case to the Department of Health by law. We are waiting for the result of abdominal ultrasonography.
# Treatment & Follow-up
No need of hospitalization, because there are no signs of hepatic failure. Prescribe analgesics for the pain. Inject Vitamin K(10 mg)subcutaneously. Statistically, he has a good chance of recovering fully. In the short term, a possible fulminant course is our major concern. We will closely monitor his liver function by scheduling a follow-up visit in a week. In the long term, the risk of persistent, progressive, chronic infection is an issue. Again, we will see him regularly to check his serological profile.
# Education
His partner should be tested for hepatitis B, and receive the hepatitis B vaccine and hepatitis B immune globulin if the result negative. We also explain to the patient how important it is for them to practice “safe sex" until the risk of infection is over.

[Conclusion]
This is a patient with acute hepatitis B. We see him on an outpatient basis, while paying attention to the fact that hepatitis B infection can be fulminant with liver failure. Educating the patient for preventing further spread of the disease is important in the follow-up.

 Delivery Indexを左から右にご一読ください。第1回において,各項目の重要性を俯瞰しました。第2回から11回までにおいて,各項目について解説を行いました。

 Deliveryにおいて最も重要な構勢は,大きな声で話す第5回),相手の目を見て話す第7回),姿勢を正して話す第8回)の3点です。「自分らしさ」を失うことなく,この3点が達成できれば,全体的な印象は大幅に改善します。

日本人のための特記事項

(1)「I'm sorry」を繰り返さない
 私たち日本人は,目上の者に対して,相槌のように「すみません」を繰り返す習慣があります。このノリで,英語圏で「I'm sorry」を繰り返すと,とても奇異に響きます。途中で思考が途絶しても,言葉がつかえても,「あやまる」必要はありません。

(2)「Ah」や「Eh」を連発しない。
 日本語の「えーと,えーと」に相当するverbal pause「ah」や「eh」は,一度や二度であれば,問題ありませんが,一文ごとに繰り返されたりすると,とても耳障りです。自分では意識していない場合がありますので,他者に評価してもらうことが大切です(第6回)。

(3)必ず結論を述べる。
 ContentsとDeliveryの双方にまたがる注意事項ですが,日本人のプレゼンテーションは,終盤が消え入るようで,いつ終わったのかわからない傾向にあります。自分の話には,自分で「落ち」をつけましょう。自分の結論を提示しないまま,相手が結論を出してくれることを期待する姿勢は,英語圏では「卑怯者」または「臆病者」と映り,信頼を得られません。常に,結論と要約を準備してから,話を始めましょう。
